Required for Participation
LIABILITY WAIVER
All participants (or their parent/guardian if a minor) must read, understand, and sign this waiver before taking part in any Hoops College training session, camp, league, or event.
1. Acknowledgment of Risk
Basketball and related athletic activities — including but not limited to skill training, conditioning, strength work, plyometrics, and live competition — involve inherent risks of injury, including muscle strains, sprains, fractures, concussions, and other physical harm. By participating in Hoops College programs, you understand and accept these risks.
2. Health & Fitness Representation
You represent that the participant is in good physical health and is capable of engaging in basketball training activities. If the participant has any medical condition, injury, or limitation that could affect participation, you agree to disclose it to Hoops College staff in writing before participating. You confirm that a physician has been consulted if you have any doubt about the participant's ability to safely participate.
3. Release of Liability
In consideration of being permitted to participate, you (and on behalf of any minor participant) hereby release, waive, and discharge Hoops College, its owners, coaches, staff, contractors, and facility partners from any and all liability, claims, demands, or causes of action arising out of or related to any loss, damage, or injury sustained while participating in Hoops College activities, except in cases of gross negligence or willful misconduct.
4. Indemnification
You agree to indemnify and hold harmless Hoops College from any claims brought by you, the participant, or anyone claiming on the participant's behalf, in connection with the participant's involvement in any Hoops College activity.
5. Emergency Medical Care
In the event of injury or medical emergency, you authorize Hoops College staff to seek and obtain medical care for the participant. You agree to be responsible for any costs associated with medical care provided.
6. Parent/Guardian Consent (Minors)
If the participant is under the age of 18, the parent or legal guardian signing this waiver represents that they have full authority to do so on the minor's behalf, and they personally accept the obligations set forth in this waiver.
7. Severability
If any portion of this waiver is found to be unenforceable, the remaining provisions remain in full force and effect.
